1. Procedure prior to an appointment<\/strong><\/h4>\n

  1. E-mail us or phone us to request an Informed Consent Form.<\/li>\n
  2. Read attentively the Informed Consent Form.<\/li>\n
  3. Contact us if you still have questions.<\/li>\n
  4. When you are ready to make an appointment, send us an e-mail to confirm that you have read the form, understand and accept the content. You bring this signed form to the appointment.<\/li>\n
  5. We will suggest some dates to you and we will set a time slot for the appointment.<\/li>\n
  6. A History Form will be e-mailed to you: you fill it in, sign it and bring to your appointment.<\/li>\n
  7. A Google map of our clinic (Barrie, Ontario) will be e-mailed to you.<\/li>\n<\/ol>\n
